At the Heart Failure and Transplant Center we provide comprehensive multidisciplinary medical and psychosocial care to patients with heart failure in order to promote and restore their quality of life.
We take a comprehensive approach to healing your heart and can help you with everything from diet and exercise to medications, device implants and even heart transplants.
We'll make sure your care plan is personalized for your needs and takes into account any other conditions you may have.

Patient Information
At Your Appointment
Your doctor will refer you for an appointment. You'll then meet with one of our attending doctors at your first visit. On follow-up visits, the entire heart failure center team will help manage your care.
Appointments for the Hospital to Home Clinic are made for patients at the hospital prior to discharge.
Nutrition Counseling
We offer counseling from a registered dietician. If interested, ask your doctor or nurse to refer you. Same-day appointments may be available.
Lab Information
Lab and Test Results
Lab results are available in MyChart, a secure online portal that lets you view lab results and communicate with your healthcare provider. You can also call us for results.
